There is no minimum order amount, however, orders less than $25.00 will be invoiced a $5.00 Small Order Fee.
Payment terms are net 30 days. Credit application may be required for terms. We accept American Express, Mastercard, Visa, and checks. All prices are in U.S. dollars. Foreign import duties and exchange rates, (if applicable) will be charged as an additional cost. All payments must be in U. S. dollars only.

If you are exempt, please e- mail or fax your tax exemption or re-sale certificate for the states we would be shipping product to. Shipping & Freight
Use the freight table found in the resource center to calculate shipping charges. UPS shipments to rural delivery locations must add $1.25 per package. Shipments to Alaska, Hawaii, Puerto Rico, U.S. Territories/possessions and areas outside the continental U.S. will be billed actual freight charges. Orders shipped Next Day Service, Second Day Service or order values over $2000.01 are billed actual UPS or motor carrier charges. Orders exceeding UPS weight and/or size limits are shipped by motor carrier and will be billed actual freight charges. If you’d prefer, we can also use your UPS or FedEx account numbers for shipping collect. International customers are responsible for applicable import duties, taxes and fees in addition to freight. We recommend UPS 2-day service.
Most UAL items are shipped from St. Charles IL within 24-48 hours of ordering. Custom labels and tags take 5-10 business days. Custom sterile labels take 4-6 weeks. Custom tapes take 10 days from proof approval.
If you wish to return because there is a quality issue or a UAL error, call for a CQT number (return authorization) and we will pick up product and issue credit or replace. No cost to you.
If you order the wrong product or you decide you don’t need, that’s not a problem. We want you to be 100% satisfied with any stock product you purchase from us. Contact us for a CQT number and send it back to us and we will credit the purchase cost (excluding freight) for the original order. The following conditions must be met:
- Product is not older than 60 days from the date of invoice.
- Product is in resalable condition-unopened boxes.
Please note that in a few instances there may be a restocking fee.
All returns for credit require authorization. If you wish to return product, contact the UAL customer service department at 1-800-423-4643 and we will provide a CQT number and information about where to return the merchandise. No returns will be accepted without a CQT number. If a return is submitted without an CQT number, product will be refused and sent back to the customer at customer's expense. Authorized returns must be received by UAL within 30 working days from the date of CQT issuance.
International Customers will be responsible for sending product back unless it is a UAL error. Custom printed or sterilized product is not returnable unless printed incorrectly or defective.
Merchandise must be returned in full unopened boxes, original packaging and in good condition. If there is any loss or damage at the time of delivery, it is essential to note it on the delivery receipt. Any concealed damage should be reported to the delivering carrier within 15 days. Keeping the entire package is necessary until after the carrier issues a concealed damage inspection report
